Sarah Price
Director of Population Health, NACHC
As Director, Clinical Integration at the National Association of Community Health Centers (NACHC), Sarah Price’s day-to-day roles include providing technical assistance for the integration of public health initiatives such as family healthy weight management, heart health, OUD/SUD, maternal/child health, nutrition, preventive health and other emerging topics. Sarah’s role also includes working with Primary Care Associations and Health Center Controlled Networks on quality improvement, assisting health centers to measure and improve performance in clinical, service, and cost aspects of health center operations. Alongside her role at NACHC, Sarah is a Public Health Advisor for the Georgetown School of Nursing. Previously, Sarah was a Nurse Care Manager/Director of Nursing for 12 years at Unity Health Care, Inc, a large health center in Washington, DC. With over 22 years of nursing experience focused on community and public health, Sarah is specialized in developing, piloting, implementing, and evaluating strategies related to interdisciplinary care services and innovative primary care workflows. Sarah earned a Master of Science in Nursing‐Education from George Mason University and a Bachelor of Science in Nursing from Emory University School of Nursing.
